Meet.

In this initial meeting, I will meet with family members separate from my meeting with the elderly loved one. This meeting is just for family/care providers. It is an opportunity for open, frank discussion about the situation and a time to hear everyone’s perspectives and concerns.

This meeting can be in-person at a family member’s home or over Zoom.

Assess.

It is helpful to meet your elderly loved one in their own home. The familiar setting allows them to be more relaxed. This time is spent engaging in social conversation, administering cognitive testing, and observing their physical abilities within the home environment. While this is only a snapshot in time, my interactions with your loved one, combined with family input, allows us to better establish what resources will be most appropriate.

Plan.

Once we have a solid assessment of the situation we can sit down and discuss a plan. Recommendations, tasks, assignments. Connect to appropriate resources
